ABSENCE NOTIFICATION FORM

Each musician is an integral participant to the success of the whole ensemble. Even one musician's absence is felt and affects the balance of the section.

Students are allowed a maximum of 3 excused absences per semester. After this point, ensemble directors will initiate a conversation with the student and parents to determine if the student will realistically be prepared to perform in the final concert.

If you plan to be absent, or have to miss a portion of rehearsal, please submit this form at least 24 hours in advance, whenever possible.
